FLU SEASON!

We have walk in and pre-bookable flu clinics at the surgery every Wednesday morning until 13th November 8.10am until 12pm. We also have a walk in clinic on Saturday 23rd November 8.30am - 12pm.

The Surgery will be closed from 12pm on the following afternoons for staff training

Cancellation of Baby Clinics

There will be no baby clinic on the following dates:

  • Wednesday 16th October 2019
  • Wednesday 20th November 2019

For medical assistance when the surgery is closed please dial 111 or visit your local pharmacy. For Urgent medical attention dial 999


 

Sick Notes

A sick note is only required after seven days of your illness. You will need to inform your employer and if necessary, complete a self certification form. This form is obtained from your employer.

A sick note from the GP is needed for an illness lasting more than five days. You will need to make an appointment to see the doctor for this. If your illness extends beyond the period certified by the doctor, you will need to see the doctor again for an extension.

Occasionally, a telephone call may be sufficient, if this has been previously agreed with the GP. If you have been discharged from the hospital and require a sick note, please supply a copy of the hospital discharge note. A sick note can be made on the basis of information on the discharge letter. Please make an appointment to see the GP if you are unsure.

Some non face to face requests for a sick note may be refused. An appointment with the GP is required if you have been asked to obtain a Med 4 certification.
