We would like to welcome Dr Bell back to the surgery from Wednesday 4th November following her maternity leave.  Dr Bell will be working on Wednesday and Fridays at the surgery.

PRESCRIPTION NOTICE

Due to the high demand in requests for prescriptions the turn around time is now 72hrs. We are working hard to process these requests as quickly as we can. Please only order if you are due to run out.

BABY CLINIC

BABY CLINICS WILL STILL BE HELD EVERY WEDNESDAY AS USUAL BUT THERE WILL BE NO HEALTH VISITOR AVAILABLE UNTIL FURTHER NOTICE AND YOU MUST BOOK AN APPOINTMENT, WALK IN APPOINTMENTS NOT AVAILABLE. WE DO ADVISE THAT PLANNED IMMUNISATIONS SHOULD GO AHEAD AND THESE WILL BE AVAILABLE FROM A GP.

Sick Notes

A sick note is only required after seven days of your illness. You will need to inform your employer and if necessary, complete a self certification form. This form is obtained from your employer.

A sick note from the GP is needed for an illness lasting more than five days. You will need to make an appointment to see the doctor for this. If your illness extends beyond the period certified by the doctor, you will need to see the doctor again for an extension.

Occasionally, a telephone call may be sufficient, if this has been previously agreed with the GP. If you have been discharged from the hospital and require a sick note, please supply a copy of the hospital discharge note. A sick note can be made on the basis of information on the discharge letter. Please make an appointment to see the GP if you are unsure.

Some non face to face requests for a sick note may be refused. An appointment with the GP is required if you have been asked to obtain a Med 4 certification.
