We are requesting  all patients attending  the surgery are to wear a face covering.  This does not have to be a clinical face mask. A scarf would be suffice as a face covering .  Your cooperation is much appreciated.

PRESCRIPTION NOTICE

Due to the high demand in requests for prescriptions the turn around time is now 72hrs. We are working hard to process these requests as quickly as we can. Please only order if you are due to run out.

BABY CLINIC

BABY CLINICS WILL STILL BE HELD EVERY WEDNESDAY AS USUAL BUT THERE WILL BE NO HEALTH VISITOR AVAILABLE UNTIL FURTHER NOTICE AND YOU MUST BOOK AN APPOINTMENT, WALK IN APPOINTMENTS NOT AVAILABLE. WE DO ADVISE THAT PLANNED IMMUNISATIONS SHOULD GO AHEAD AND THESE WILL BE AVAILABLE FROM A GP.

New Patient Registration

To register with the practice, please click and print off the registration form GMS1, and patient questionnaire complete it and bring it to the practice together with 2 forms of identification.

If you are not sure that you live within the practice boundaries, please telephone the surgery.

Identification

To register yourself as a patient with this Practice we require you to provide us with proof of identity (ID). Failure to provide this evidence may result in the Practice being unable to accept your registration.

Whenever possible, photographic ID should be provided: Passport, Drivers Licence Company ID card (or similar) and also a recent utility bill or bank statement with your address.

Only if you do not have one of the above we will accept other forms of identity – 2 of the following:

  • NHS Medical Card
  • Utility Bills or Account Statements – as proof of address
  • Council Tax Bills – as proof of address

Children

For children whose parents are registered within the Practice, we will not need proof of identity.

Nursing Homes

Nursing homes will not be required to show new patient ID.

Overseas Nationals or Asylum Seekers

  • Passport/Visa
  • Home Office Documentation OR UK Border Agency Papers
  • National Insurance Card
  • Letter from Benefits Agency OR Benefit Book OR Signing On card
  • Paid Utility Bills – as proof of address

When you come to the surgery, please ask for copy of our patient information leaflet. 

Additionally, we would be grateful if you could complete a patient questionnaire Adult Questionnaire and/or a Child Questionnaire for each member of the family, these are also available from reception. This would provide useful for the doctors or nurses as your records may take some time to be transferred from your previous practice.

If you are on regular repeat medications, it may be useful to make an appointment with a GP to have them sorted onto our system.

If you move house please let us know your new address. You may need to register with another practice if your new address is outside our practice area.
