What I Do
I now work as part of the home visiting team for the PCN. My main role is assessing and treating housebound patients with acute illness.
This often includes management of social and chronic issues and liaising with GPs and the social prescribing team within the PCN to ensure a holistic care approach for our patients.
How I Support Patients
I strive to give the best care possible to the patients I see, and my aim is to create a happy, shared learning, interactive and inclusive working environment within the team.
