Sick Notes, Fit Notes and Self-Certification
You may need a sick or ‘fit’ note if you have been off work for 7 days or more.
To request a sick note please contact us online it is the quickest way to submit your request and allows you to submit all the information yourself on the request form. You can do this during our working days between 08.00 am and 6.30 pm
IMPORTANT INFORMATION
From 1 October 2026, if you need time off work because of a hospital stay, an A&E visit, an operation or a clinic appointment, the hospital team treating you should give you a fit note (sick note). It should cover the whole time they expect you to be off. Please ask for it before you leave the hospital. This follows national guidance, saves you a seperate trip to the practice and keeps our appointments free for patients who need them.

If you’ve been off for 7 days or fewer
If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill.
Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.
Find out more about sick notes on the NHS website. Please note this link may not work if you are using a mobile telephone to access this page.
