Sick Notes, Fit Notes and Self-Certification

You may need a sick or ‘fit’ note if you have been off work for 7 days or more.

To request a sick note please contact us online it is the quickest way to submit your request and allows you to submit all the information yourself on the request form. You can do this during our working days between 7.00am – 12.00pm and 1.30pm to 3.30pm using the link below

If you’ve been off for 7 days or fewer

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill.

Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.
