- A family member or carer can fill in the brief form on your behalf
- Ring the practice in the usual way and our reception team will fill in the form on your behalf.
- Please note- walk in patients will be given a paper copy of the form to fill in and this will be added to the triage list in time order to ensure equity and fairness for patient access.
