What I Do
I provide quality care for patients, especially those who are housebound or elderly.
Patients are often relieved to know a healthcare professional can visit them at home rather than them having to attend the GP surgery.
How I Support Patients
As well as we annual diabetes or hypertension reviews, I give advice on lifestyle and healthy living.
I can refer to other services if needed, ie Social Prescribing. Social Services.
I like to give quality time for a conversation as many patients live alone, and may have few or no visitors.
