Introduction

I have been employed as a Patient Care Navigator since October 2023.  This is a new role  at the practice to help support our GPs in the move to a patient triage system.  I help to assist the Duty GP team with the incoming triage requests and on the advice of the GP I will liaise with the patients to request further medical information or send out information on behalf of the GPs such as an appointment booking link.  I also support the reception and admin team with various tasks.
