What I Do
My main role is to support patients with life outside of their GP practice, and to connect them with non-medical services to improve their quality of life.
I link patients to relevant services and provide personalised care, supporting people with social, emotional and practical elements of their lives.
How I Support Patients
Support from the SPLW team aims to provide patients with the correct support and alleviate the pressures on GP surgeries when it comes to helping people with non-medical issues.
I receive referrals from other members of staff when eligible patients require support. Their referrals are triaged and then I make contact with the patient to see how best I can help.
